Anyone intending to be on campus on any given day MUST complete a COVID-19 self-assessment form before they arrive.

Please note:
  • To be allowed entry into CCNM, staff, students, faculty and visitors must either print the results of the form or display the results on their cell phone
  • Residents are required to complete the self-assessment form every time they leave their residence floor
  • If the recommendation is that you get tested, you are not to enter the College under any circumstances. CCNM students are required to contact Dr. Jonathan Prousky, ND at jprousky@ccnm.edu immediately.
